ABSTRACT

Purpose: To evaluate the quality of breast reconstruction service at a university hospital, as assessed by the patients. Methods: This cross-sectional study enrolled adult women who underwent immediate or delayed breast reconstruction by any technique performed at a university hospital between 1 and 24 months before the assessment. The Brazilian version of the Health Service Quality Scale (HSQS) was self-applied to the participants. The HSQS produces percentage scores, which are expressed in values ranging from 0 to 10 for each domain of the scale, and into an overall percentage quality score. The management team was asked to establish a minimum satisfactory score for the breast reconstruction service. Results: Ninety patients were included. The management team considered 8.00 the minimum satisfactory score for the service. The overall percentage score was 93.3%. Only one domain, 'Support,' had an average score lower than that considered satisfactory (7.22 ± 3.0); while the others had higher scores. The domain that scored highest was 'Qualification' (9.94 ± 0.3), followed by 'Result' (9.86 ± 0.4). There was a positive correlation between 'type of oncologic surgery' and 'intentions of loyalty to the service' (ρ = 0.272; p = 0.009) and a negative correlation between 'education' and 'quality of the environment' (ρ = ­0.218; p = 0.039). The higher the patient's level of education, the higher the score attributed to 'relationship' (ρ = 0.261; p = 0.013) and the lower the score of 'aesthetics and functionality' (ρ = ­0.237; p = 0.024). Conclusion: The quality of the breast reconstruction service was considered satisfactory, but there is a demand for structural improvements, better interpersonal relationships, and a stronger support network for patients.

ABSTRACT

O estudo analisa os arranjos regionais de governança do Sistema Único de Saúde (SUS), segundo esfera jurídica dos prestadores e distribuição espacial da produção de serviços de média e alta complexidade no Brasil. Tais arranjos expressam o modo como a reforma do Estado e do sistema de saúde promoveram a redistribuição de funções entre entes governamentais e privados no território. Realizou-se estudo exploratório com base em dados secundários de abrangência nacional, do biênio 2015-2016. Por meio da análise de agrupamentos baseada na composição dos percentuais da produção dos principais prestadores, foram classificadas 438 regiões de saúde. Na assistência de média complexidade, predominou o prestador público municipal (ambulatorial) e o prestador privado filantrópico (hospitalar). Na alta complexidade, predominou o prestador filantrópico e lucrativo (ambulatorial e hospitalar). A produção de média complexidade foi registrada em todas as regiões de saúde, porém, em 12 estados, mais da metade dela está concentrada em apenas uma região de saúde. A produção de alta complexidade é concentrada nas regiões das capitais estaduais. Os arranjos de governança podem ser mais ou menos diversos e desiguais, se considerados os diferentes segmentos e níveis de concentração regional da produção de média e alta complexidade. O estudo sugere que a convergência entre descentralização e mercantilização favoreceu o reescalonamento da função de prestação de serviços, com ampliação da escala de atuação de prestadores privados e fortalecimento dos municípios polos. As características dos arranjos de governança desafiam a regionalização do SUS orientada pelas necessidades coletivas das populações.


El estudio analiza las modalidades regionales de gestión en el Sistema Único de Salud (SUS), según la categoría jurídica de los prestadores y la distribución espacial para la provisión de servicios de media y alta complejidad en Brasil. Tales modalidades expresan el modo mediante el cual la reforma del Estado y del sistema de salud promovieron la redistribución de funciones entre entes gubernamentales y privados en el territorio nacional. Se realizó un estudio exploratorio, basado en datos secundarios de alcance nacional, durante el bienio 2015-2016. Mediante un análisis de agrupamientos, basado en la composición de porcentajes relacionados con la provisión de servicios de los principales prestadores, se clasificaron 438 regiones de salud. En la asistencia de media complejidad, predominó el prestador público municipal (ambulatorio) y el prestador privado filantrópico (hospitalario). En la alta complejidad, predominó el prestador filantrópico y lucrativo (ambulatorio y hospitalario). La provisión de media complejidad se registró en todas las regiones de salud, sin embargo, en 12 estados, más de la mitad de la misma está concentrada en sólo una región de salud. La producción de alta complejidad está concentrada en las regiones de las capitales de los estados. Las modalidades de gestión pueden ser más o menos diversas y desiguales, si se consideran los diferentes segmentos y niveles de concentración regional en la provisión de servicios de media y alta complejidad. El estudio sugiere que la convergencia entre descentralización y mercantilización favoreció el reescalonamiento de la función de prestación de servicios, con una ampliación de la escala de actuación de prestadores privados y el fortalecimiento de los municipios más importantes. Las características de las modalidades de gestión desafían la regionalización del SUS, orientada por las necesidades colectivas de las poblaciones.


The study analyzes regional Brazilian Unified National Health System (SUS, in Portuguese) governance arrangements according to providers' legal sphere and the spacial provision of middle and high-complexity services. These arrangements express the way in which State and health system reforms promoted the redistribution of functions between governmental and private entities in the territory. We carried out an exploratory study based on national-scope secondary data from 2015-2016. Using cluster analysis based on the composition of the provision percentages of the main providers, we classified 438 health regions. In middle-complexity health care, municipal public providers (outpatient) and private philanthropic providers (hospital) predominate. In high complexity provision, philanthropic and for-profit providers (outpatient and hospital) predominate. Middle-complexity provision was recorded in all health regions. However, in 12 states, more than half of the provision is concentrated in only one health region. High-complexity provision is concentrated in state capital regions. Governance arrangements may be more or less diverse and unequal, if different segments and regional concentration levels of middle and high-complexity provision are considered. The study suggests that the convergence between decentralization and mercantilization favored re-scaling of service provision, with increase in the scale of participation of private providers and strengthening of reference municipalities. Governance arrangement characteristics challenge SUS regionalization guided by the collective needs of the population.

RESUMO Objetivo: realizar a adaptação cultural e a validação semântica do Standard Precautions Questionnaire para profissionais de saúde brasileiros. Métodos: estudo desenvolvido entre 2017 e 2018 no Rio de Janeiro. A adaptação cultural incluiu a tradução, o consenso sobre a versão em português, a avaliação por especialistas, a retrotradução, o consenso e a comparação com a versão original, a avaliação semântica e a validação semântica. Todos os aspectos éticos foram respeitados. Resultados: na validação de conteúdo, as pontuações variaram de 0.60 a 1.00 e a pontuação foi igual a 0.96, indicando que os itens são muito representativos. Na avaliação semântica, 22 (80%) itens foram considerados relevantes e 24 (100%) itens foram considerados claros e compreensíveis por todos os profissionais. Conclusão: os itens da versão brasileira do instrumento foram representativos e relevantes para a prática clínica de profissionais de saúde na avaliação dos obstáculos para aderir a precauções padrão. Estudos são necessários para avaliar as propriedades psicométricas do instrumento.


RESUMEN: Objetivo: Efectuar la adaptación cultural y la validación semántica del Standard Precautions Questionnaire para profesionales de salud brasileños. Método: Estudio desarrollado entre 2017 y 2018, en Río de Janeiro. La adaptación cultural comprendió: traducción, consenso sobre la versión en portugués, evaluación de expertos, retrotraducción, consenso y comparación con la versión original, evaluación semántica y validación semántica. Se respetaron todos los aspectos éticos. Resultados: Para la validación de contenido, las puntuaciones oscilaron entre 0,60 y 1,00 y el puntaje fue de 0,96, indicando que los ítems son muy representativos. En la evaluación semántica, 22 (80%) ítems fueron considerados relevantes y 24 (100%) ítems fueron considerados claros y comprensibles por todos los profesionales. Conclusión: Los ítems de la versión brasileña del instrumento fueron representativos, relevantes en la práctica clínica del profesional de salud para evaluar los obstáculos de adhesión a las precauciones estándar. Serán necesarios estudios evaluando las propiedades psicométricas del instrumento.


ABSTRACT Objective: to accomplish the cultural adaptation and semantic validation of the Standard Precautions Questionnaire for Brazilian health professionals. The cultural adaptation included translation, consensus on the Portuguese version, evaluation by experts, back-translation, consensus and comparison with the original version, semantic evaluation and semantic validation. In the semantic evaluation, 22 (80%) of the items were considered relevant and 24 (100%) of the items were considered clear and understandable by all professionals. Conclusion: the items of the Brazilian version of the instrument were representative, relevant for the clinical practice of health professionals in assessing the obstacles to adhere to standard precautions. Studies are needed to evaluate the psychometric properties of the instrument.

ABSTRACT

O Conselho Nacional de Secretários de Saúde ­ CONASS vem desenvolvendo desde 2012 uma linha de atuação intitulada CONASS Debate, que fomenta a discussão de temas importantes para o setor, na intenção de vocalizar para a sociedade as posições técnicas e políticas das Secretarias de Saúde dos Estados e do DF, produzir e difundir conhecimento, além de disseminar informações, inovar e incentivar a troca de experiências e de boas práticas. Este livro aborda, nos três capítulos que o compõem, o tema governança, a partir de diferentes análises, sem a pretensão de encerrar a discussão sobre o tema, mas de contribuir trazendo elementos para o debate. O primeiro capítulo apresenta uma análise da Governança que é realizada em um ambiente de redes, com seus componentes que reúnem elementos políticos, jurídicos, socioeconômicos, níveis de interdependência, compartilhamento de riscos e processos, e propõe um possível modelo analítico sobre Governança Colaborativa, considerando os aspectos contratual, processual e relacional para a obtenção do resultado esperado de evolução da rede e aplicação prática em redes já estabelecidas. É o resultado de ampla pesquisa bibliográfica, para a qual o CONASS contou com o apoio da Fundação Dom Cabral, realizada com foco na compreensão dos principais conceitos de governança, seus componentes, e na evolução do tema como área de estudo, conforme a metodologia descrita no seu anexo. O segundo capítulo discorre sobre Governança Regional das Redes de Atenção à Saúde, tema bastante atual na agenda dos gestores do Sistema Único de Saúde (SUS) que procuram uma gestão eficiente, organizando o modelo de atenção nas regiões de saúde para assegurar ao usuário a continuidade do cuidado em saúde. Apresenta o tema tratado em termos de modelos, de sua institucionalidade, de seus sistemas gerenciais e de seu sistema de financiamento. O terceiro capítulo apresenta a experiência do Laboratório de Inovação em Governança da Rede Mãe Paranaense, uma parceria do CONASS com a Secretaria Estadual de Saúde do Paraná (SES/PR), em curso na Macrorregião Noroeste onde se agrupam cinco regiões de saúde do estado (11ª ­ Campo Mourão; 12ª ­ Umuarama; 13ª ­ Cianorte; 14ª ­ Paranavaí; e 15ª ­ Maringá). A Rede Mãe Paranaense foi organizada em razão da necessidade de melhorar o acesso e a qualidade da atenção à saúde prestada ao cidadão e enfrentar o fenômeno das condições crônicas que representam mais de 76% da carga de doença no Paraná e tem produzido resultados importantes a partir da implantação de ideias inovadoras como a organização do Comitê Executivo Macrorregional da Rede, mudança de conceitos e processos de trabalho nas equipes de saúde. Os temas desenvolvidos nos três capítulos serviram de base para as apresentações nas mesas da 7ª edição do CONASS Debate, evento realizado em Brasília no dia 23 de novembro de 2016, reunindo os secretários estaduais de saúde, equipes técnicas dos estados e autoridades do Ministério da Saúde. Dessa forma, apresentamos esta edição reafirmando nosso intuito de contribuir para a ampliação desse debate, ao trazer elementos que se articulam e cooperam para o trabalho urgente de implantação e funcionamento das Redes de Atenção à Saúde em todas as regiões do país.

ABSTRACT

O presente trabalho apresenta a proposta de um Programa para o desenvolvimento do Sistema Nacional de Inovação de Produtos Médicos  –   SNIPM no âmbito do Sistema Único de Saúde  – SUS, de forma integrada e alinhada com as ações do Ministério da Saúde. Os conhecimentos e as ferramentas em engenharia clínica permitiram realizar uma análise diferenciada do contexto socioeconômico do setor, a sistematização e a percepção dos fatores e atores que induzem ou retardam o crescimento tecnológico do setor. Neste artigo, buscou-se contextualizar o setor de produtos médicos no Brasil à luz da atual conjuntura socioeconômica nacional e mundial, caracterizando-o como de grande indutor de atividades inovadoras. Posteriormente, são abordadas as definições e teorias sobre os Sistemas Nacionais de Inovação e as especificidades do SNIPM no Brasil, apresentando seus atores incluindo suas atribuições e competências. A metodologia utilizada para a coleta e análises de dados econômicos é apresentada, de modo a caracterizar a importância deste segmento para a economia nacional e suas potencialidades na indução do surgimento de tecnologias inovadoras no SNIPM. A análise dos dados setoriais coletados durante a pesquisa é apresentada, como o objetivo de quantificar e apresentar os números socioeconômicos do setor de produtos. Por fim, é apresentado o delineamento de um Programa destinado acatalisar o desenvolvimento do SNIPM considerando as demandas tecnológicas e prioridades do SUS e as dos mercados-alvo do setor produtivo. A proposta, que foi desenvolvida e implementada no Ministério da Saúde, pode se constituir como uma das ferramentas no âmbito do SUS destinadas a fortalecer e modernizar o setor de equipamentos e materiais de uso em saúde, visando à ampliação da capacidade inovadora das empresas e mudança do seu patamar competitivo.


This paper presents a proposal of a program for the development of the Brazilian National Innovation System for Medical Products  (SNIPM) under the National Health System  (SUS) integrated and aligned with the actions of the Brazilian Ministry of Health. Clinical engineering knowledge and tools allowed performing a differentiated socio-economic analysis of the sector, the systematization and the perception of actors and factors that induce or retard the growth of the technology sector. In this article, the medical devices sector in Brazil was first contextualized to in the light of current national and global socio-economic conjuncture, in order to characterize it as a major inducer of innovative activities. The definitions and theories about the National Innovation Systems are described, followed by a discussion of the specifics of SNIPM in Brazil, with its actors, including their duties and responsibilities. The methodology for the collection and analysis of economic data, were shown, in order to characterize the importance of this sector to the national economy and its potential to induce the rising of innovative technologies whithin the SNIPM. The analysis of industry data is presented, which attempts to present and comment on some industry data collected during a survey. In the Results section, it is presented the design of a program used to to catalyze the development of the SNIPM considering the technological demands and priorities of the SUS and the target markets of the productive sector. The proposed program, which was developed and implemented within the Brazilian Ministry of Health, can become one of the tools within the SUS to strengthen and modernize the sector of medical equipments and materials, aiming to increase the innovative capacity of firms and change its competitive standing.

ABSTRACT

INTRODUÇÃO: Um dos grandes problemas encontrados nas instituições públicas é a desmotivação dos funcionários, impossibilitando a realização de um atendimento humano, com qualidade e, em alguns casos, com dignidade ao paciente. O referencial teórico baseia-se nos estudos de Elton Mayo sobre a produtividade do homem, que é influenciada por fatores psicológicos e que pode aumentar quando os trabalhadores percebem que a direção da organização dá atenção a eles. Outra referência consiste no paradigma da dádiva de Marcel Mauss que, em oposição ao utilitarismo prático e econômico em que se encontra o século XXI, com uma visão mercantil de dar e pagar, propõe uma sociedade lógica não utilitarista, baseada em uma relação tripartite de dar, receber e retribuir. OBJETIVO: Este artigo aborda a política de gestão de pessoas nos serviços de saúde, ressaltando a importância da humanização, valorização e motivação do servidor público na cidade de Belém-Pa. MATERIAL E MÉTODO: Este estudo combinou pesquisa bibliográfica e documental, investigação qualitativa por meio de estudo de caso na FHCGV, fazendo uso de grupos focais e de entrevistas estruturadas. O exame dos dados coletados ocorreu por meio da técnica de análise do conteúdo. CONCLUSÃO: Conclui-se que os servidores não conhecem em sua totalidade as ações de humanização e valorização adotadas pela FHCGV. Os principais aspectos que motivam os funcionários são: comemoração de datas festivas, amizades de trabalho, boa relação com a chefia, reconhecimento do trabalho executado, preocupação com a qualidade, capacitação, realização profissional, gratificação financeira pelo desenvolvimento institucional e o ticket alimentação. As ações de humanização adotadas pela FHCGV não proporcionaram mudanças efetivas no exercício laboral diário, devido a diversas trocas de gestão, em função da mudança de governo, ocasionando uma redução no incentivo às práticas de humanização, que passaram a ser retomadas desde 2009, porém não tendo ainda a devida divulgação.


INTRODUCTION: One of the major problems encountered in public institutions is the lack of motivation among employees, hindering the realization of a human service, quality and, in some cases, the patient with dignity. The theoretical framework is based on studies of Elton Mayo on productivity of man, which is influenced by psychological factors and can increase when employees perceive that the organization's management pays attention to them. Another reference is the paradigm of the gift of Marcel Mauss, in opposition to utilitarianism practical and economical it is in the twenty-first century, with a vision to provide commercial and pay proposes a society not utilitarian logic, based on a tripartite relationship of giving receive and reciprocate. OBJECTIVE: This article discusses the management policy of people in the health services, emphasizing the importance of humanization, appreciation and motivation of public servants in the city of Belém-Pa. MATERIAL AND METHODS: This study combined documentary and bibliographical research, qualitative research through case study in FHCGV, using focus groups and structured interviews. CONCLUSION: The survey data was collected by means of the technique of content analysis. We conclude that the servers do not know in full humanization actions and valuation adopted by FHCGV. The main aspects that motivate employees are celebrating festive dates, friends work, good relationship with the leadership, recognition of work performed, concern for quality, training, professional fulfillment, financial reward for developing institutional and meal ticket. Humanization actions taken by FHCGV provided no effective change in exercise daily labor, due to several changes of management, due to the change of government, causing a reduction in the incentives for humanization practices, which are now incorporated since 2009, but not yet having proper disclosure.

ABSTRACT

Os objetivos deste artigo são: descrever o processo de reforma sanitßria ocorrido em países da América Latina e suas repercussões nos sistemas de saúde do Brasil, da Colômbia e do Peru; analisar a conformação dos sistemas nacionais de saúde desses três países fronteiriços; e avaliar os limites e possibilidades de uma integração dos sistemas locais de saúde no contexto da tríplice fronteira amazônica. A partir de documentos e bases de dados oficiais, faz-se uma anßlise do desenvolvimento dos sistemas de saúde e da organização da rede de serviços nesse segmento de fronteira localizado em plena região amazônica. Na comparação entre os três países, constata-se que o modelo adotado pelo Brasil tem as características de um sistema nacional de saúde de finalidade pública, enquanto na Colômbia e no Peru vigoram sistemas de asseguramento, com adoção de planos obrigatórios de assistência, segmentação dos usußrios e contingenciamento da cobertura. Os três países convergem quanto à mescla de público e privado na provisão de serviços, mas diferem no nível de acesso e no tipo de regulação por parte do Estado. Conclui-se que as especificidades dos sistemas de saúde dos três países fronteiriços tornam complexo o desafio da construção de um sistema único de saúde na fronteira que possa garantir a universalidade do acesso, a equidade no atendimento e a integralidade da assistência. Apesar das discrepâncias identificadas, porém, constata-se que a preponderância do setor público na rede de atenção bßsica permite vislumbrar a possibilidade de integração dos sistemas locais de saúde no que concerne à oferta organizada de ações programßticas de saúde do primeiro e segundo níveis de assistência.


The objectives of this article are to describe the health sector reform process in Latin American countries and its impact on the health systems of Brazil, Colombia, and Peru; analyze the structure of the national health systems of these bordering nations; and assess the constraints and potential for integrating the local health systems along the triple Amazon border. Using official documents and databases, we analyzed health system development and the structure of the services network in this segment of the border located in the heart of the Amazon region. Comparison of the three countries confirmed that Brazil's model has the characteristics of a national public health system, while the Colombian and Peruvian models emphasize insurance systems, with the adoption of compulsory health care plans, the segmentation of users, and limitations on coverage. The three countries are similar in their adoption of a public-private mix in service delivery, but they differ in the degree of access and type of State regulation. We concluded that the characteristics of the health systems in the three countries make the challenge of building a unified health system capable of guaranteeing universal access, equity in care, and comprehensive care a complex undertaking. However, despite the differences identified, we can state that the weight of the public sector in the basic care network makes it possible to foresee integration of the local health systems to provide an organized supply of programmatic health actions at the primary and secondary level of care.

ABSTRACT

En este artículo se describen las condiciones de salud de la población salvadoreña y, con mayor detalle, el sistema de salud de El Salvador, incluyendo su estructura y cobertura, sus fuentes de financiamiento, los recursos físicos, materiales y humanos con los que cuenta, las actividades de rectoría que desarrolla el Ministerio de Salud Pública y Asistencia Social, y la participación de los usuarios de los servicios de salud en la evaluación del sistema. Asimismo se discuten las más recientes innovaciones implantadas por el sistema salvadoreño de salud, dentro de las que destacan la aprobación de la Ley de Creación del Sistema Nacional de Salud que busca ampliar la cobertura, disminuir las desigualdades y mejorar la coordinación de las instituciones públicas de salud.


This paper describes the health conditions in El Salvador and the main característics of the Salvadoran health system, including its structure and coverage, its financial sources, the physical, material and human resources available, the stewardship functions developed by the Ministry of Public Health, and the participation of health care users in the evaluation of the system. It also discusses the most recent policy innovations including the approval of the Law for the Creation of the National Health System, which intends to expand coverage, reduce health inequalities and improve the coordination of public health institutions.

ABSTRACT

En este artículo se describe el sistema de salud de Argentina, que está compuesto por tres sectores: público, de seguridad social y privado. El sector público está integrado por los ministerios nacional y provincial, y la red de hospitales y centros de salud públicos que prestan atención gratuita a toda persona que lo demande, fundamentalmente a personas sin seguridad social y sin capacidad de pago. Se financia con recursos fiscales y recibe pagos ocasionales de parte del sistema de seguridad social cuando atiende a sus afiliados. El sector del seguro social obligatorio está organizado en torno a las Obras Sociales (OS), que aseguran y prestan servicios a los trabajadores y sus familias. La mayoría de las OS operan a través de contratos con prestadores privados y se financian con contribuciones de los trabajadores y patronales. El sector privado está conformado por profesionales de la salud y establecimientos que atienden a demandantes individuales, a los beneficiarios de las OS y de los seguros privados. Este sector también incluye entidades de seguro voluntario llamadas Empresas de Medicina Prepaga que se financian sobre todo con primas que pagan las familias y/o las empresas. En este trabajo también se describen las innovaciones recientes en el sistema de salud, incluyendo el Programa Remediar.


This paper describes the health system of Argentina.This system has three sectors: public, social security and private.The public sector includes the national and provincial ministries as well as the network of public hospitals and primary health care units which provide care to the poor and uninsured population. This sector is financed with taxes and payments made by social security beneficiaries that use public health care facilities. The social security sector or Obras Sociales (OS) covers all workers of the formal economy and their families. Most OS operate through contracts with private providers and are financed with payroll contributions of employers and employees. Finally, the private sector includes all those private providers offering services to individuals, OS beneficiaries and all those with private health insurance.This sector also includes private insurance agencies called Prepaid Medicine Enterprises, financed mostly through premiums paid by families and/or employers.This paper also discusses some of the recent innovations implemented in Argentina, including the program Remediar.

ABSTRACT

En este trabajo se describen las características generales del sistema de salud de Bolivia: su organización y cobertura; sus fuentes de financiamiento y gasto en salud; los recursos físicos, materiales y humanos de los que dispone; las actividades de rectoría que desarrolla, y el nivel que ha alcanzado la investigación en salud. También se discuten las innovaciones más recientes que se han llevado a cabo en los últimos años, incluyendo el Seguro Universal Materno Infantil, el Programa de Extensión de Cobertura a Áreas Rurales, el Modelo de Salud Familiar, Comunitaria e Intercultural y el programa de subsidios monetarios Juana Azurduy, dirigido a fortalecer la atención prenatal y del parto.


This paper describes the Bolivian health system, including its structure and organization, its financing sources, its health expenditure, its physical, material and humans resources, its stewardship activities and the its health research institutions. It also discusses the most recent policy innovations developed in Bolivia: the Maternal and Child Universal Insurance, the Program for the Extension of Coverage to Rural Areas, the Family, Community and Inter-Cultural Health Model and the cash-transfer program Juana Azurduy intended to strengthen maternal and child care.

En este trabajo se describe el sistema de salud de Brasil, que está compuesto por un sector público que cubre alrededor de 75 por ciento de la población y un creciente sector privado que ofrece atención a la salud al restante 25 por ciento de los brasileños. El sector público está constituido por el Sistema Único de Salud (SUS) y su financiamiento proviene de impuestos generales y contribuciones sociales recaudadas por los tres niveles de gobierno (federal, estatal y municipal). El SUS presta servicios de manera descentralizada a través de sus redes de clínicas, hospitales y otro tipo de instalaciones, y a través de contratos con establecimientos privados. El SUS es además responsable de la coordinación del sector público. El sector privado está conformado por un sistema de esquemas de aseguramiento conocido como Salud Suplementaria financiado con recursos de las empresas y/o las familias: la medicina de grupo (empresas y familias), las cooperativas médicas, los llamados Planes Autoadministrados (empresas) y los planes de seguros de salud individuales. También existen consultorios, hospitales, clínicas y laboratorios privados que funcionan sobre la base de pagos de bolsillo, que utilizan sobre todo la población de mayores ingresos. En este trabajo se analizan los recursos con los que cuenta el sistema, las actividades de rectoría que se desarrollan y las innovaciones más recientemente implantadas, incluyendo el Programa de Salud de la Familia y el Programa Más Salud.


This paper describes the Brazilian health system, which includes a public sector covering almost 75 percent of the population and an expanding private sector offering health services to the rest of the population. The public sector is organized around the Sistema Único de Saúde (SUS) and it is financed with general taxes and social contributions collected by the three levels of government (federal, state and municipal). SUS provides health care through a decentralized network of clinics, hospitals and other establishments, as well as through contracts with private providers. SUS is also responsible for the coordination of the public sector. The private sector includes a system of insurance schemes known as Supplementary Health which is financed by employers and/or households: group medicine (companies and households), medical cooperatives, the so called Self-Administered Plans (companies) and individual insurance plans.The private sector also includes clinics, hospitals and laboratories offering services on out-of-pocket basis mostly used by the high-income population. This paper also describes the resources of the system, the stewardship activities developed by the Ministry of Health and other actors, and the most recent policy innovations implemented in Brazil, including the programs saúde da Familia and Mais Saúde.

En este trabajo se describe el sistema de salud de Chile, incluyendo su estructura, financiamiento, beneficiarios y recursos físicos, materiales y humanos de los que dispone. Este sistema está compuesto por dos sectores, público y privado. El sector público está formado por todos los organismos que constituyen el Sistema Nacional de Servicios de Salud y cubre aproximadamente a 70 por ciento de la población, incluyendo a los pobres del campo y las ciudades, la clase media baja y los jubilados, así como los profesionales y técnicos. El sector privado cubre aproximadamente a 17.5 por ciento de la población perteneciente a los grupos sociales de mayores ingresos. Un pequeño sector de la población, perteneciente a la clase alta, realiza pagos directos de bolsillo a proveedores privados de servicios de atención a la salud. Alrededor de 10 por ciento de la población está cubierta por otras agencias públicas, fundamentalmente los Servicios de Salud de las Fuerzas Armadas. Recientemente el sistema se reformó creando el Régimen General de Garantías en Salud, que establece un Sistema Universal con Garantías Explícitas que se tradujo, en 2005, en el Plan de Acceso Universal con Garantías Explícitas (AUGE), que garantiza el acceso oportuno a servicios de calidad para 56 problemas de salud, incluyendo cáncer en niños, cáncer de mama, trastornos isquémicos del corazón, VIH/SIDA y diabetes.


This paper describes the Chilean health system, including its structure, financing, beneficiaries, and its physical, material and human resources. This system has two sectors, public and private. The public sector comprises all the organisms that constitute the National System of Health Services, which covers 70 percent of the population, including the rural and urban poor, the low middle-class, the retired, and the self-employed professionals and technicians.The private sector covers 17.5 percent of the population, mostly the upper middle-class and the high-income population. A small proportion of the population uses private health services and pays for them out-of-pocket. Around l0 percent of the population is covered by other public agencies, basically the Health Services for the Armed Forces. The system was recently reformed with the establishment of a Universal System of Explicit Entitlements, which operates through a Universal Plan of Explicit Entitlements (AUGE), which guarantees timely access to treatment for 56 health problems, including cancer in children, breast cancer, ischaemic heart disease, HIV/AIDS and diabetes.

En este trabajo se presenta una breve descripción de las condiciones de salud de Colombia y una descripción detallada del sistema colombiano de salud. Esta última incluye una descripción de su estructura y cobertura, sus fuentes de financiamiento, el gasto en salud, los recursos con los que cuenta, quién vigila y evalúa al sector salud y qué herramientas de participación tienen los usuarios. Dentro de las innovaciones más recientes del sistema se incluyen las modificaciones al Plan Obligatorio de Salud y a los montos de la unidad de pago por capitación, la integración vertical entre empresas promotoras de salud y las instituciones prestadoras de servicios, así como el establecimiento de nuevas fuentes de recursos para lograr la universalidad e igualar los planes de beneficios entre los distintos regímenes.


This document briefly describes the health conditions of the Colombian population and, in more detail, the characteristics of the Colombian health system. The description of the system includes its structure and coverage; financing sources; expenditure in health; physical material and human resources available; monitoring and evaluation procedures; and mechanisms through which the population participates in the evaluation of the system. Salient among the most recent innovations implemented in the Colombian health system are the modification of the Compulsory Health Plan and the capitation payment unit, the vertical integration of the health promotion enterprises and the institutions in charge of the provision of services and the mobilization of additional resources to meet the objectives of universal coverage and the homologation of health benefits among health regimes.

En este trabajo se describe el sistema de salud de Costa Rica, que presta servicios de salud, agua y saneamiento. El componente de servicios de salud incluye un sector público y uno privado. El sector público está dominado por la Caja Costarricense de Seguro Social (CCSS), institución autónoma encargada del financiamiento, compra y prestación de la mayoría de los servicios personales. La CCSS se financia con contribuciones de los afiliados, los empleadores y el Estado, y administra tres regímenes: el seguro de enfermedad y maternidad, el seguro de invalidez, vejez y muerte, y el régimen no contributivo. La CCSS presta servicios en sus propias instalaciones o contrata prestadores del sector privado con los que establece contratos denominados "compromisos de gestión". El sector privado comprende una amplia red de prestadores que ofrecen servicios ambulatorios y de especialidad con fines lucrativos. Estos servicios se financian sobre todo con pagos de bolsillo, pero también con primas de seguros privados. El Ministerio de Salud es el rector del sistema y como tal cumple con funciones de dirección política, regulación sanitaria, direccionamiento de la investigación y desarrollo tecnológico. Dentro de las innovaciones relativamente recientes que se han implantado en Costa Rica destacan la implantación de los equipos básicos de atención integral de salud (EBAIS), la desconcentración de los hospitales y clínicas públicos, la introducción de los acuerdos de gestión y la creación de las Juntas de Salud.


This paper describes the Costa Rican health system which provides health, water and sanitation services. The health component of the system includes a public and a private sector. The public sector is dominated by the Caja Costarricense de Seguro Social (CCSS), an autonomous institution in charge of financing, purchasing and delivering most of the personal health services in Costa Rica. CCSS is financed with contributions of the affiliates, employers and the state, and manages three regimes: maternity and illness insurance, disability, old age and death insurance, and a non-contributive regime. CCSS provides services in its own facilities but also contracts with private providers. The private sector includes a broad set of services offering ambulatory and hospital care. These services are financed mostly out-of-pocket, but also with private insurance premiums. The Ministry of Health is the steward of the system, in charge of strategic planning, sanitary regulation, and research and technology development. Among the recent policy innovations we can mention the establishment of the basic teams for comprehensive health care (EBAIS), the de-concentration of hospitals and public clinics, the introduction of management agreements and the creation of the Health Boards.

En este trabajo se describen las condiciones de salud de Cuba y el sistema cubano de salud, incluyendo su estructura y cobertura, sus fuentes de financiamiento, su gasto en salud, los recursos físicos, materiales y humanos de los que dispone, y las actividades de rectoría e investigación que desarrolla. También se discute la importancia de sus instituciones de investigación y se describe el papel de los usuarios de los servicios en la operación y evaluación del sistema, así como las actividades que en este sentido desarrollan la Federación de Mujeres Cubanas y los Comités de Defensa de la Revolución. La parte final de este trabajo se dedica a discutir las innovaciones más recientes dentro de las que destacan las redes de cardiología, la Misión Milagro y la Batalla de Ideas.


This paper describes the health conditions in Cuba and the general characteristics of the Cuban health system, including its structure and coverage, its financial sources, its health expenditure, its physical, material and human resources, and its stewardship functions. It also discusses the increasing importance of its research institutions and the role played by its users in the operation and evaluation of the system. Salient among the social actors involved in the health sector are the Cuban Women Federation and the Committees for the Defense of the Revolution. The paper concludes with the discussion of the most recent innovations implemented in the Cuban health system, including the cardiology networks, the Miracle Mission (Misión Milagro) and the Battle of Ideas (Batalla de Ideas).

En este trabajo se describen las condiciones de salud en Ecuador y, con mayor detalle, las características del sistema ecuatoriano de salud, incluyendo su estructura y cobertura, sus fuentes de financiamiento, los recursos físicos, materiales y humanos de los que dispone, las tareas de rectoría que desarrolla el Ministerio de Salud Pública, la generación de información en salud, las tareas de investigación, y la participación de los ciudadanos en la operación y evaluación del sistema. También se discuten las innovaciones más recientes que se han implantado en el sistema ecuatoriano de salud dentro de las que destaca la incorporación de un capítulo específico sobre salud a la nueva Constitución que reconoce a la protección de la salud como un derecho humano y la construcción de la Red Pública Integral de Salud.


This paper describes the health conditions in Ecuador and, in more detail, the characteristics of the Ecuadorian health system, including its structure and coverage, its financial sources, the physical, material and human resources available, and the stewardship activities developed by the Ministry of Public Health. It also describes the structure and content of its health information system, and the participation of citizens in the operation and evaluation of the health system. The paper ends with a discussion of the most recent policy innovations implemented in the Ecuadorian system, including the incorporation of a chapter on health into the new Constitution which recognizes the protection of health as a human right, and the construction of the Comprehensive Public Health Network.

ABSTRACT

En este trabajo se describen las condiciones de salud y el sistema de salud de Guatemala, incluyendo su estructura y cobertura, sus fuentes de financiamiento, las actividades de rectoría que en él se desarrollan, así como las tareas de generación de información en salud e investigación. También se discuten los esfuerzos por ampliar la cobertura de servicios básicos, sobre todo a las comunidades rurales pobres. Destacan dentro de las innovaciones recientes del sistema guatemalteco de salud el Programa de Extensión de Cobertura de Servicios Básicos y el Programa de Accesibilidad de Medicamentos, así como los acuerdos del Ministerio de Salud con organizaciones de la sociedad civil para prestar servicios básicos en comunidades rurales.


This paper describes the health conditions in Guatemala and, in more detail, the characteristics of the Guatemalan health system, including its structure en coverage, its financial sources, the stewardship functions developed by the Ministry of Health, as well as the generation of health information and the development of research activities. It also discusses the recent efforts to extend coverage of essential health services, mostly to poor rural areas.The most recent innovations also discussed in this paper include the Program for the Expansion of Coverage of Essential Services, the Program to Expand Access to Essential Drugs and the agreements between the Ministry of Health and several non-governmental organizations to provide essential services in rural settings.

ABSTRACT

En este trabajo se describe el sistema de salud de Honduras, incluyendo los retos que enfrenta, su estructura y cobertura, su financiamiento, los recursos con los que cuenta y las actividades de rectoría que en él se desarrollan. Este sistema cuenta con un sector público constituido por la Secretaría de Salud (SS) y el Instituto Hondureño de Seguridad Social (IHSS), y un sector privado en el que predomina el uso de servicios con pago de bolsillo. El Plan Nacional de Salud 2010-2014 se plantea una ambiciosa reforma del sistema que contempla la universalización del aseguramiento mediante un sistema de salud integrado y plural, en el que la SS funcionaría como una institución rectora y reguladora, y que contaría con un seguro público nacional de salud para la población pobre y un IHSS que funcionaría exclusivamente como aseguradora al contratar servicios para sus afiliados con proveedores públicos y privados, que operarían bajo un modelo de salud familiar y comunitaria.


This paper describes the health system of Honduras, including its challenges, structure coverage, sources of financing, resources and stewardship activities. This system counts with a public and a private sector. The public sector includes the Ministry of Health (MH) and the Honduran Social Security Institute (HSSI). The private sector is dominated by a set of providers offering services payed mostly out-of-pocket. The National Health Plan 2010-2014 includes a set of reforms oriented towards the creation of an integrated and plural system headed by the MH in its stewardship role. It also anticipates the creation of a public health insurance for the poor population and the transformation of the HSSI into a public insurance agency which contracts services for its affiliates with public and private providers under a family medicine model.

ABSTRACT

En este trabajo se describe el sistema mexicano de salud. En la primera parte se discuten las condiciones de salud de la población mexicana, con énfasis en los padecimientos emergentes que constituyen las principales causas de muerte: diabetes, enfermedades isquémicas del corazón, padecimientos cerebrovasculares y cáncer. En la segunda parte se describe la estructura básica del sistema: las instituciones que lo conforman, la población a la que cubre, los beneficios que reciben los afiliados de las distintas instituciones, las fuentes de financiamiento del sistema, los niveles de protección financiera alcanzados, los recursos físicos, materiales y humanos con los que cuenta y las principales tareas de rectoría que desarrollan la Secretaría de Salud y otros actores. También se describen los mecanismos de participación de los ciudadanos en la vigilancia y evaluación del desempeño del sistema y su nivel de satisfacción con los servicios recibidos. La tercera parte se dedica a discutir las innovaciones más recientes y su impacto en la operación del sistema. Destaca la discusión del recién creado Sistema de Protección Social en Salud y su brazo operativo, el Seguro Popular de Salud. El trabajo concluye con un análisis de los principales retos que enfrenta en el corto y mediano plazo el sistema mexicano de salud.


This paper describes the Mexican health system. In part one, the health conditions of the Mexican population are discussed, with emphasis in those emerging diseases that are now the main causes of death, both in men and women: diabetes, ischaemic heart disease, cerebrovascular diseases and cancer. Part two is devoted to the description of the basic structure of the system: its main institutions, the population coverage, the health benefits of those affiliated to the different heath institutions, its financial sources, the levels of financial protection in health, the availability of physical, material and human resources for health, and the stewardship functions displayed by the Ministry of Health and other actors. This part also discusses the role of citizens in the monitorization and evaluation of the health system, as well as the levels of satisfaction with the rendered health services. In part three the most recent innovations and its impact on the performance of the health system are discussed. Salient among them are the System of Social Protection in Health and the Popular Health Insurance. The paper concludes with a brief analysis of the short- and middle-term challenges faced by the Mexican health system.

En este artículo se describen las condiciones de salud de Nicaragua y las características de su sistema de salud, incluyendo su estructura y cobertura, sus fuentes de financiamiento, los recursos físicos, materiales y humanos con los que cuenta, las actividades de rectoría que desarrolla el Ministerio de Salud, los mecanismos de participación ciudadana en la gestión y evaluación de los sistemas de salud, y la satisfacción de los usuarios con los servicios recibidos. También se discuten las innovaciones más recientes, dentro de las que destacan la promulgación de una nueva Ley General de Salud, la descentralización de la regulación de los establecimientos de salud y el diseño de un nuevo modelo de atención a la salud denominado Modelo de Salud Familiar y Comunitario.


This paper describes the health conditions in Nicaragua and discusses the characteristics of its national health system including its structure and coverage, its financial sources its physical, material and human resources the stewardship functions developed by the Ministry of Health the participation of citizens in the operation and evaluation of the system and the level of satisfaction of health care users. It also discusses the most recent policy innovations, including the new General Health Law, the decentralization of the regulation of health facilities and the design and implementation of a new health care model known as Family and Community Health Model.
